We believe in transparency when it comes to the cost of care. Our goal is to help you understand your insurance benefits and financial responsibilities before services are rendered, so there are no surprises and no unnecessary financial hardship.
Insurance Plans Accepted
We participate with many major insurance plans. Coverage and benefits vary by plan and carrier. Because insurance policies differ, acceptance of insurance does not guarantee full coverage of services.
If your plan is out-of-network, services may still be available. Our team will review available options with you and explain how out-of-network benefits work prior to your visit.
Please note: Patients are ultimately responsible for verifying their insurance benefits.

Understanding Your Financial Responsibility
Depending on your insurance plan, you may be responsible for one or more of the following:

Copayment (Copay):
A fixed amount due at the time of service, as determined by your insurance plan. -
Coinsurance:
A percentage of the total cost of services that you are responsible for after your insurance processes the claim. -
Deductible:
The amount you must pay out-of-pocket before your insurance begins covering services. -
Non-Covered Services:
Some services may not be covered by your insurance and may result in full patient responsibility.
All copayments, coinsurance, and deductible amounts are due at the time of service, unless prior arrangements have been made.
Financial Acknowledgement & Intake Process
As part of our intake process, all patients are provided with a "Financial Acknowledgement" Form. These forms clearly outline:

We encourage patients to review the form carefully and ask questions so we can work together to ensure care remains accessible and manageable.
Out-of-Network Services
If we are out-of-network with your insurance plan:

You may be responsible for a higher portion of the visit cost

Payment may be required at the time of service

A superbill can be provided upon request for submission to your insurance carrier

Reimbursement, if any, is determined solely by your insurance plan
